Appointment policy
A $31 deposit reserves your visit.
New patients pay a $31 deposit when scheduling. The deposit secures your dedicated appointment time and is applied to the cost of your visit.
If you need to reschedule or cancel, please provide at least 48 hours’ notice. Deposits are fully refundable with at least 48 hours’ notice. A cancellation with less notice or a missed appointment may result in forfeiture of the deposit.

Embark Dental is in-network with the providers below. The team also accepts several other insurance plans and will file claims on the patient’s behalf.
We do not accept Medicaid or file Medicaid claims.
